Mariner Coast Guard physicals in Forest Park, IL
We perform your Mariner Coast Guard physical and complete the medical practitioner sections of the official CG-719K form. Exams are by appointment at our Forest Park location and take about 45 minutes.
What the exam includes
Each Mariner Coast Guard physical covers the exams and tests recorded on the CG-719K.
Medical history review
We go over the medical conditions and current medications you list on your form.
General physical exam
A full physical exam, including height, weight, pulse, and blood pressure.
Vision testing
Visual acuity with and without correction, field of vision, and color vision testing with color plates.
Hearing screening
A whisper test. If there is a concern about your hearing, Coast Guard rules require a referral to an audiologist or hearing specialist for formal testing.
Physical ability
A demonstration of the physical tasks listed on the CG-719K.
CG-719K completion
We complete the medical practitioner sections, and your examiner signs the form.
Coast Guard regulations allow a licensed nurse practitioner to perform, witness, or review the required tests and exams (46 CFR 10.302). The exam is $150, card or cash. Drug testing is available on site through the clinic for a separate fee.

Here is what happens from booking to exam day.

Pick your time and complete three short forms: privacy practices, a card on file, and a brief intake.
Check your email
You'll receive your CG-719K link and the full medical intake from Carepatron.
Fill out your sections
Complete the applicant sections of the CG-719K and bring the form with you.
Come in for your exam
Plan on about 45 minutes. We complete the medical sections, and your examiner signs the form.
What to bring
- Your CG-719K with the applicant sections already completed. Download the current form from the National Maritime Center.
- Photo ID issued by the government
- Your glasses or contacts, if you wear them
- A list of your current medications
Your form has to be in hand. The exam can't be completed without it.

Do you take insurance?
No. The exam is $150 cash pay, by card or cash.
Where is the exam?
By appointment at the DOTDOC clinic location, 7503 Roosevelt Rd, Forest Park, IL 60130.
Can I get my drug test at the same visit?
Yes. The clinic offers drug testing on site, so you can do the physical and the drug test in one stop. The drug test is priced separately from the $150 exam; ask for current pricing when you arrive.
What form do you use?
Every Mariner Coast Guard physical is completed on the official CG-719K, the Coast Guard's Application for Medical Certificate.
Do I need to fill out the CG-719K before I come?
Yes. Download the current form from the National Maritime Center, complete the applicant sections, and bring it with you. A link also arrives by email after you book.

Can a nurse practitioner do my Coast Guard physical?
Yes. Under 46 CFR 10.302, the required tests and exams may be performed, witnessed, or reviewed by a licensed medical doctor, physician assistant, nurse practitioner, or designated medical examiner.

The U.S. Coast Guard. We document the exam findings on your CG-719K, and the Coast Guard makes the final decision on medical qualification, waivers, and your credential.

This Notice explains how medical information about you may be used and disclosed and how you can obtain access to it.

We may also use or disclose PHI for legally permitted public health and safety activities, abuse or neglect reporting, research, organ donation, coroners or funeral directors, workers' compensation, health oversight, law enforcement, government functions, and court or administrative proceedings.
Stricter confidentiality laws
We follow stricter federal or Illinois protections when they apply, including protections for mental health records, records about developmental disabilities, information related to HIV, genetic information, records of certain services that minors may consent to on their own, and substance use disorder patient records.
To the extent we have substance use disorder patient records subject to 42 CFR part 2, we will not use or disclose information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and subpoena as required by law.
Our responsibilities
We must protect the privacy and security of PHI, notify you of a breach when required, follow the Notice currently in effect, and avoid uses or disclosures not described in the Notice unless you authorize them or law permits them.

Website and emergencies
Website content is general information, not medical advice. Contacting us does not create a relationship between you and a clinician or confirm an appointment. If you have a medical emergency, call 911 or go to the nearest emergency department.
Appointments and payment
An appointment is confirmed when Carepatron or Enhanced Health issues a confirmation. The Mariner Coast Guard physical is cash pay, by card or cash. The confirmed price controls.
Cancellations
Please provide at least 24 hours' notice to cancel or reschedule. There is currently no fee for late cancellation. If you miss your appointment without notice, you are charged the full scheduled visit price.
Mariner Coast Guard physicals
Mariner examinations document findings accurately for credentialing. Enhanced Health does not guarantee medical qualification, form acceptance, waiver approval, credential issuance, or any decision by the U.S. Coast Guard. You must bring your CG-719K with the applicant sections completed; the exam cannot be completed without it.
Drug testing
Drug testing at the exam location is provided by the clinic, not by Enhanced Health PLLC. It is priced and billed separately by the clinic.
Safety, third parties, and privacy
You must provide accurate information and follow any preparation instructions. Carepatron, the exam location, credentialing authorities, and other third parties have their own terms and responsibilities. The Website Privacy Policy governs Website information, and the Notice of Privacy Practices governs PHI.
Legal terms
Illinois law governs these Terms, and Cook County courts are the designated venue where legally permitted. Nothing in the Terms waives patient rights or limits liability that cannot lawfully be waived or limited.
