Coast Guard Mariner Physicals

Need your CG-719K completed for a merchant mariner credential, original, renewal, or raise of grade? I complete the full medical evaluation and the paperwork, and you walk out with your exam done right the first time. Rejected paperwork costs mariners weeks. Mine gets filled out completely, legibly, and to Coast Guard requirements.

  1. Book your exam.
    Book online in about a minute, or call or text 312-244-5681. Most mariners get seen within a few days.
  2. Come in to Forest Park with your completed CG-719K.
    Plan for about 45 minutes at 7503 Roosevelt Rd. Vision, hearing, physical exam, and the medical examiner portion of your form finished on site.
  3. Leave with your paperwork.
    Signed, complete, and ready to submit with your application.

  • Your CG-719K form, applicant sections already completed and in hand. Exams cannot be finished without it
  • Government-issued photo ID
  • Glasses or contacts, if you wear them
  • A list of your current medications and any specialist letters
Exam location: exams are performed by appointment at the DOTDOC clinic, 7503 Roosevelt Rd, Forest Park, IL 60130. Our administrative office is separate; please do not go to the Oak Park address for exams.

In-Home Lab Draws & Specimen Collection

Skip the lab lobby. I bring the draw to your home or office anywhere within 25 miles of Oak Park, IL, for one flat $99. That covers the visit, the venipuncture, and getting your specimen labeled, packaged, and handled to your laboratory's exact requirements. Testing itself is performed by the lab on your order.

  2. I come to you.
    Home, workplace, or senior community. Calm, unhurried, and pediatric-friendly.
  3. Your specimen goes to the lab.
    Collected, prepped, and shipped or delivered per your lab's requirements. Results come from your ordering provider.

I collect blood draws, fingerstick and dried blood spot cards, urine, saliva, stool kits, buccal swabs, and most provider-ordered specialty kits, including kits ordered through platforms such as Rupa Health.

One thing I'm asked a lot: yes, I'm a nurse practitioner, but this service is collection only. I don't order labs, add tests, or interpret results through it. Your order has to come from your own provider, or arrive as a lab kit, and your results go back to whoever ordered them. If you need labs ordered, start with your provider, then bring me the order.

CPR Certification, Brought to Your Team

No sending your staff across town one at a time. I bring American Heart Association training to your daycare, office, or clinic, and everyone leaves certified the same day. Groups of 6 or more.

    Weekday, evening, or weekend. I work around nap schedules and shift changes.
  2. I train your team on-site.
    Hands-on practice with manikins and AED trainers, at your location.
  3. Everyone leaves certified.
    AHA eCards issued after course completion, valid for two years.

Built for childcare providers: pediatric CPR, AED, choking, and the first aid module, with certification cards for your licensing file. Directors, ask about training your whole staff in one visit.

Directors: I also run a pediatric house call practice, so I know exactly what DCFS monitors look for in your training files. Ask about staff-wide certification days.

Care that comes to where you are

I'm ShaTara Coleman, FNP-BC, a Board-Certified Family Nurse Practitioner and the founder of Enhanced Health PLLC. I've spent my career caring for patients across the lifespan, and I built this practice on a conviction that hasn't changed since day one: healthcare should meet people where they are. Sometimes that's a mariner who needs his credential exam done right so he can get back on the water. Sometimes it's a grandmother who can't easily get to a lab. Sometimes it's a daycare director who needs her whole staff certified before a licensing visit.

This practice is faith-built and mission-led. My aim with every visit is the same: to make quality healthcare more accessible, one patient, one family, and one community at a time.

FAQ

Do you take insurance?
These services are cash pay: card, cash, and most HSA/FSA cards. That's what keeps pricing flat and scheduling fast. For lab draws, the laboratory may bill your insurance separately for the testing itself, depending on your order.
Where do the mariner physicals happen?
By appointment at the DOTDOC clinic location, 7503 Roosevelt Rd, Forest Park, IL 60130. Our Oak Park address is an administrative office only; no patients are seen there.
Do I need a provider order for a lab draw?
Yes. Specimens are collected only with a valid provider order or a laboratory collection kit, unless otherwise permitted by law. I don't perform the testing; your designated laboratory does, and results come from your ordering provider.
You're a nurse practitioner, so can't you just order my labs?
Fair question, and the answer is no, not through this service. This service line is specimen collection only. Ordering tests, choosing panels, and interpreting results are part of a treatment relationship with your own provider, and that stays with them. It keeps your care coordinated in one place, with the person who knows your history and will act on the results. So the order comes from your provider or your mailed kit, and I handle the draw from there.
What if my lab mailed me a collection kit?
I collect most provider-issued kits, including functional medicine and specialty kits. Text me the kit name before you book so I can confirm its collection requirements.
How far will you travel for a home draw?
Within 25 miles of Oak Park, IL for the flat $99. Use the ZIP checker above, and if you're just outside the line, reach out anyway. I'll tell you honestly whether I can make it work.
What if my blood draw can't be completed?
It happens, and it's usually nobody's fault. Dehydration, small or rolling veins, recent illness, cold hands, and certain medications can all make veins hard to access on a given day. Following standard clinical practice, I make no more than two gentle attempts in a single visit; going past that isn't good care. If a draw can't be completed, the visit fee still applies, since the travel and time occurred, and I'll schedule a return visit at the reduced $49 rate so we can try again when you're better prepared.
How do I give my draw the best chance of success?
Hydrate. Unless your order requires restricting fluids, drink water steadily the day before and the morning of your draw; water is allowed with almost all fasting orders. Keep your arms warm before I arrive, skip heavy caffeine that morning if you can, and tell me up front if you're historically a hard stick so I can plan the site and equipment accordingly.
How many people do you need for a CPR class?
Six or more at your location. Smaller teams are welcome to combine with a neighboring business or center; text me and I'll help coordinate.
Are your CPR cards accepted by employers and DCFS?
The courses are American Heart Association courses, BLS for Healthcare Providers and Heartsaver Pediatric First Aid CPR AED, with AHA eCards issued after completion. AHA certification is the most widely accepted in the country. Always confirm your specific employer's or licensor's requirements.
Do you see children?
Yes. Pediatric lab draws are done with a calm, family-centered approach, and a parent or legal guardian must be present.
